Creating the Space for Healing and Wellness

We believe every patient deserves to be treated with compassionate care. Our team has carefully designed everything from your first office call to your new patient exam, to gain a thorough understanding of your approach to healing and wellness.


Items to Complete Before Your First Visit

Prior to your first appointment, our team will help you organize and complete the following information:


  • A completed onPatient Portal Profile, via a website link that will be emailed to new patients prior to their appointment.
  • A pathology report. We ask patients to fax these documents if they are coming from a location outside of the Beaumont Health System. 
  • The last 3 years of breast imaging reports. These include all mammograms, US, MRI, and biopsy reports.

Items to Bring on Your First Visit

To help your first visit go as smoothly as possible, please bring these important items on the day of your appointment:

  • If you are coming from a hospital outside of the Beaumont Health System, please bring a CD that includes the last 3 years of breast imaging reports. These include all mammograms, US, MRI, and biopsy reports.
  • A Photo ID (you cannot be seen without a Photo ID).
  • Your current Health Insurance information. If you are using Blue Care Network insurance, then we will need a global referral form.

Having these prepared and ready for your appointment will save you time and allow us to make your first visit the very best experience possible. While we do our best to stay on time, there are unexpected delays that occur. Please plan on at least 1 hour for your visit.
